This is (in my opinion) an improved version of the Level Up menu that is better organized and easier to use. The menu is basically organized by skill (Strength, Perception, etc.) with perks in increasing skill requirements below the skill. In other words, Iron Fist requires Strength 1, Big Leagues requires Strength 2, Armorer requires Strength 3, and so on.

Vanilla > LevelUpMenuEx

Hmmm... I think I prefer vanilla. I can see where someone might prefer this, however, especially if you were adding additional perks.
