Dawnstar. Stage 1 vampirism. Getting attacked everywhere.

Just started the dawnstar quests and became a vampire. I have Sarena following me. I just left to go talk to inns and carts. Anyways, I went into town and everyone is attacking me. I went to a small farm. They attacked me. 

I read that stage 4 vampirism causes this and to just feed. I did and I'm still getting attacked. Since that's the only suggestion I can find, I'm assuming it has to do with one of the mods, but I'm not finding any that would do anything like this. 

Not sure what else to do here. If I get attacked every time I'm in town, I'll be reloading and going to the dawnstar. Any ideas? Am I missing a mod setting or is there a vanilla fix?
