All books have a missing graphic on page 1

Just completed the default STEP install, and notice that anytime I open a book, the small graphic at the top left of page one is missing, with a rainbow colored placeholder (and the word "graphic) in its place.

 

I'm guessing I messed up a mod install along the way. I'm thinking possibly Better Book Names.

 

What's the best way I can diagnose this problem?
